The patient is responsible for their deductible, co-insurance and/or co-pay which is dictated by their individual insurance plan, not by our practice
Our practice will bill your primary insurance. As a courtesy, we will bill your secondary insurance ONCE. If the claim is not paid, it becomes the patients responsibility.
If your insurance policy is an HMO which requires your Primary Care Provider (PCP) to approve an appointment with a specialist, our office will submit the request to your PCP’s office. However, it is ultimately the responsibility of the patient to ensure your appointment is authorized prior to appointment date.
